I wonder if you have any examples (codes) to achieve communication from forms 6i with a card reader.

Don't have any, sorry.

However, check this OTN Forms forum's discussion. A "card reader" should be considered as any "device connected to a serial port" (or USB, never mind that - it also supports serial communication). It appears that you should use Java bean(s). Also, I believe (based on reading about Oracle for the past several years) that Francois Degrelle's blog contains code you might find useful.
