I don't understand the question.

what does phone number format have to do with Oracle SQL or PL/SQL?
specify to who, where & for what?

1/ Do NOT use IM/SMS speak
2/ Always post your Oracle version, with 4 decimals.
3/ With any SQL or PL/SQL question, please, Post a working Test case: create table and insert statements along with the result you want with these data then we will be able work with your table and data. Explain with words and sentences the rules that lead to this result.

Regards
Michel

Ok then, it's a forms question. There is an item property called format mask. you do not need a trigger.
